Early Weight Issues Emerging Before Primary School

An increasing number of Malaysian children are experiencing weight-related health issues even before entering primary school, raising concerns about their overall development.

According to Dr Ng Hui Ying, a consultant paediatrician at Oriental Melaka Straits Medical Centre, childhood obesity is becoming more evident among preschool and primary school children, often affecting sleep quality, attention span and learning ability.

She noted that the issue is frequently overlooked, as discussions about weight remain sensitive for many families, delaying early screening and intervention.

Growing Prevalence Among Children

Data shows a steady rise in childhood obesity both globally and locally. The American Academy of Pediatrics reports that millions of children and adolescents are affected worldwide, making obesity one of the most common chronic conditions.

In Malaysia, studies indicate that obesity rates among children have increased significantly over the past decades, with a substantial proportion now classified as overweight or obese.

Dr Ng warned that early-onset obesity – occurring even before primary school – is becoming more common and requires timely attention.

Health Risks and Warning Signs

Medical professionals assess children’s weight using body mass index (BMI) charts based on age and growth standards. Children who are identified as overweight or obese may require further evaluation, particularly if risk factors are present.

Warning signs can include high blood pressure, darkened skin patches, excessive thirst, frequent urination, snoring, daytime sleepiness and difficulty concentrating.

Children may face higher risks if there is a family history of conditions such as diabetes, hypertension or heart disease, or if there were complications during pregnancy or birth.

If left unaddressed, obesity can lead to long-term health issues, including heart disease, diabetes, liver complications and emotional challenges.

 

Impact on Mental Health and School Life

Beyond physical health, research shows that children with excess weight are more likely to experience bullying, as well as anxiety and depression.

These challenges can affect their confidence, social interactions and ability to focus in school, further impacting their learning outcomes.

 

Lifestyle Factors Driving the Trend

Dr Ng said most cases are linked to lifestyle habits shaped by a child’s environment, although genetic and medical factors may also play a role.

Key contributors include diet, physical inactivity and excessive screen time. Studies cited by the American Academy of Pediatrics indicate that children who spend more than two hours a day on screens face a significantly higher risk of obesity.

Screen exposure is also associated with unhealthy eating patterns, reduced physical activity and shorter sleep duration.

Local findings highlight similar concerns, with many children consuming high levels of sugar and fat, skipping balanced meals and maintaining irregular eating habits.

 

Role of Families and Early Intervention

Dr Ng emphasised that family habits play a crucial role in shaping children’s health. Encouraging balanced nutrition, regular physical activity and consistent routines can help children develop healthier behaviours.

She added that routine paediatric check-ups are important for monitoring growth, identifying potential issues early and planning appropriate interventions.

“Obesity is a chronic condition that requires continuous care,” she said, stressing that families should be actively involved in making sustainable lifestyle changes.

Early detection and consistent family support, she noted, can help prevent long-term health complications and support children’s overall well-being.
